Depending on what you're submitting, we may need different information:
We’ll need to know about the service you’ve been provided with and the date it happened.
We’ll need to know about the practitioner: their name, where their offices are located, and if they have a provider license number.
Depending on what’s been done, we may also need you to submit a claims form with more information. A claims representative will let you know what we need.
When submitting a disability claim, statements are required from the plan member, plan sponsor, and attending physician with support provided by the disability case manager.
