Depending on what happened, we may need different information from you during the claims process:
We’ll need to know the full name and date of birth of the person covered, their SIN or policy number, and details about their cause of death.
We’ll need your contact information, relationship to the covered person, and any estate documentation.
You’ll need to provide a death certificate and, depending on the situation, either police or coroner reports or any additional claims forms as requested by your claims representative.
You may need to fill out a claims form to send back to your claims representative.
