Make a claim as a beneficiary
Contact us at 1-800-454-8061 (option 4) or individual_life_claims@cooperators.ca, or connect with your Financial Advisor to initiate the process.
Based on the discussion with your Claims representative, you may be asked to complete a specific form(s). If so, you can find links to the form(s) below.
Please do not fill out any forms until requested by your Claims representative.
Claimant Statement (to be completed by you, as the beneficiary)
Physician Statement (to be completed by the deceased’s last attending physician)
a consent to collect more information (to be completed by you, as the beneficiary)

If you need to make changes to your policy, such as:
- Change your address
- Name a new beneficiary
