What you need to know about Student Eligibility forms
To ensure that students continue to receive the benefits they need, it’s important for plan members to provide current information on student status prior to dependants beginning their post-secondary education. If student eligibility forms are not received, dependents who are full-time students may experience an interruption in their coverage.
When should plan members fill out the form?
Plan members can find the Student Eligibility form on Benefits Now®. For each enrolled student, plan members must fill out the form:
- Prior to the dependant beginning their post-secondary education
- When the dependent has completed their post-secondary education or no longer meets the definition of a full-time student
For consistency with other administrative practices, it’s no longer required for plan members to fill out the form annually. You only need to notify us in the event a plan member’s situation changes.
You or your plan member can email completed student eligibility forms to student_verification@cooperators.ca. You can manage student eligibility by using the self-service dependent reporting available on Benefits Now® for Plan Sponsors.
